Click on Renew Patient Portal button below to request an intake appointment.  Select "request appointment" once on the portal site.  

 

You may request a therapist, time, date and location to begin but you don't have to.  Our staff will reach out to discuss options and openings.  Our response time is normally within 24 hours of your request on week days.  If you do not hear from us, please email us.  We will work hard to get you quickly matched and an initial intake appointment.

NEW PATIENT INTAKE Appointment requests must be made 48 hours in advance.  Please let us know if you have more than one insurance policy.  Commercial Policies REQUIRE a credit card on file before you will receive an appointment. Cards are charged at the time of the appointment or shortly after.  Please contact your insurance company so that you understand your benefits for behavioral health services.  

 

All forms must be completed on our patient portal prior to your intake appointment.  If not completed, that appointment could be canceled.  Please reach out to our office if you are struggling with the portal forms.  Remember to "scroll down" to sign all forms.
